This survey is designed to measure the over all system reliability of HRA’s delivery of benefits to all currently eligable reciepents.
The survey is based on the idea that we can use multiple personal mean time between failures (MTBF) to accurately extrapolate to the BEST POSSIBLE performence (i.e. the longest possible MTBF) of the entire system to draw useful insights to how reliable the system is as a whole.
Please report ONLY passive catastrophic failures that meet the following definition (see worked example for real world thresholds):
Passive – No action (or inaction) by the client could of altered the outcome of the event/failure
Catastrophic – Less then 24 hour immediate threat to food, shelter and/or healthcare
Failure – No “rational person” would consider the outcome to be correct, fair and/or to even make sense
